The consent form for the 2009 Apex Extreme has now been finalised and is available online to download. It is essential that each Scout taking part in the competition brings a completed form to the start of the event. Those without one will not be able to participate.
This year’s form has been completely redesigned and is now spread across two pages due to extra requirements from the Army team who will be running an assault course that forms part of Sunday’s activities. Please ensure that both pages of your consent form are stapled together.
If you had already downloaded one of the old-style single sheet consent forms, please discard it, as this will not satisfy the requirements of the competition.
Those Scouts who are taking part in the event and are over 18 and from the Scout Network DO still need to complete a consent form. In their case they can complete the form and sign it themselves.
