Consent forms
Every team member must bring a completed consent form with them when they sign in at the start of the Apex Challenge weekend. Anybody without a form will not be allowed to take part.
Download your form below. If you are under 18 your form must be signed by a parent or carer. If you are 18 or over you can sign the form yourself.
Please bring all your team’s forms to sign in at Event Centre when you arrive. We cannot sign any of your team in until we have consent forms for all team members. It is also helpful if each team signs in individually, rather than arriving in a large group of three or four teams all at once.

Adult information forms
All adults on site, either as a helper or a Scout leader needs to fill in and sign an adult information form which needs to be handed in at Event Centre when you arrive.
